To Set up MailBox Via Fax, follow the instruction below:

1. Download PS Form 1583 and Fill out ALL information
2. Make Copy of Driver License and Passport (If you don't have a Passport, see PS Form 1583 for other acceptable identification or call us for more information.)
3. Download Credit Authorization Form, fill out ALL information
4. Copy of your credit card
5. Fax ALL above required pages to 909-752-8828, and call us as soon as your fax went through.
6. We will assign you a mailbox right over the phone.
